Online register

According to Dutch law, it is compulsory for everyone to register with a GP in their own place of residence. You can decide for yourself which GP you are going to.


What should you do ?
Sign up through the registration form.                                                  
Your registration is completed when you have received  a confirmation email that you are registred.

At your first visit you bring your identity card and insurance card. You can show this at the frontoffice.


What else do you need to do?
When exchanging general practice you need a correct registration, but also the deregistration from your previous GP must be well organized.
 
We ask you to contact your own doctor to indicate that you have found a new GP. The medical file will then be sent electronically to us by your previous GP.
